Journey Church Connection Form

You will notice that there are some required fields in the form below. The purpose of those required fields is to make sure that the routing of this Connection Card is sent to the correct profile in our Planning Center app which we use to connect with our church.

Any information on this form will be matched up with your profile in Planning Center. Any existing information will be overwritten by the information you provide on the form, so be sure to review the form before you click Submit.

Please know that we will not release your information to anyone other than the authorized personnel selected by our leadership for the purpose of connecting you to your family here at the Journey Church. We will use your information to ensure you are connected to your Journey Church family and that you have the information you need to remain connected.
